Community Care Coordination and Education

The Community Resource Coordinator serves as the bridge between community members and the resources available to them.

  • Coordination of Community Health Services
  • Assist with care coordination
  • Knowledgeable of services, programs, and opportunities for both tribal & non-tribal community members

Hospital Follow-up

Community Health has resources if you are hospitalized. Contact us if you need support advocating for yourself with your hospital team. We also assist with care coordination and the management of follow-up appointments. Our transportation team can also help with transportation to medical appointments following your hospital stay.

Eligibility Requirements:

Tulalip community member within Snohomish County admitted to an area hospital in need of discharge planning, transition planning, advocacy, and resources.
